The District and the colleges will use the short, self-check health questionnaire provided by Cleared4. The questionnaire is accessible via any phone, tablet or laptop connected to the internet. You will receive a text or email reminder with a personal link to take the survey.
All students may be required to show their Clear Pass to authorized personnel upon request. We are working with Cleared4 to resolve this issue as well as some other technical issues. The District received reports that individuals with no medical insurance are not given the option to write in the field that they do not have medical insurance as previously instructed.
As a work-around, Cleared4 recommends that the following steps be followed to report that you currently do not have medical insurance. On a blank piece of paper, write the following statement: " I, first name, last name, DOB attest that I do not have medical insurance. All nine colleges and the District office will have testing sites available on a staggered schedule and you will be able to make an appointment at any site. However, once you have successfully accessed the Cleared4 site to create an account, please contact the Cleared4 Help Desk at covidcolleges biocept.
